The exemptions process for the COVID-19 Vaccine Mandate ended on 9/10/21.

If you have an approved vaccine exemption, you are required to have weekly COVID-19 testing.  Weekly emails are sent to those with approved exemptions with instructions for on campus testing. If you are tested ON campus, you do NOT need to upload anything. 

If you wish to be tested off campus, you can find a testing location here. You must then follow these instructions:

Off site testing:
You are able to get your weekly COVID-19 tests at another location as long as:
  1. It is a  COVID-19 molecular PCR test conducted by a healthcare provider or laboratory (at home and rapid testing is not accepted).
  2. You upload your result on a weekly basis to the GHC secure portal by attaching the result to a secure message, send a message to the nurse.
  3. Your test result  must clearly state your name, date of birth, date of test, and the results. 
  4. Testing should be no fewer than 5 days between, and no more than 9 days after the last test.

Vaccine Mandate for Winter 2022:

  • If you haven't already done so, please upload your completed COVID-19 vaccine record by December 13. If you've already uploaded your vaccine status, you do NOT need to do it again.

Vaccine Upload: 
All students, faculty and staff please upload your vaccination status to the Graham Health Center secure patient portal.

The university will not share individual vaccination status publicly, only aggregate data used to track the collective vaccination rate of all campus community members. To learn more about uploading vaccination status, visit the patient portal instructions page.

Housing Requirements and uploading your COVID-19 Immunization Card

Please read our instructions on how to upload your vaccine information.

Please allow up to 5 business days for processing. Once GHC reviews and accepts your COVID-19 vaccine record, the details of your vaccine will be viewable in the IMMUNIZATION section of the portal.

If you still have questions after 5 days, please send a message via the secure patient portal.  This will expedite a reply. Thank you for your patience.

COVID-19 Vaccine Requirement - University Housing*

If you will not be fully vaccinated (2 weeks after completion of the vaccine series) for COVID-19 the day prior to move in, you must apply for exemption by contacting the Dean of Students office at: or (248) 370-3352. If you have additional questions can be sent to the email above.

Moderna and Johnson & Johnson COVID-19 Vaccine is now available by appointment at GHC. Additional dates will be added as they become available.
