RM Forms and Checklist

Risk Management Insurance Review/Modify/Waive Form - This form should be completed for the following situations:

• Review Certificate of Insurance and/or Insurance limits in contract
• Request a modification of insurance requirements
• Waiver of Insurance
• Request an Oakland University Certificate of Insurance

Golf Cart Driving Agreement Form - Departments must have all golf cart drivers complete the Agreement Form every two (2) years or when you have a new golf cart driver request.

Risk Management On-Campus Accident/Incident - This form should be completed for On-campus accidents/injuries within twenty-four (24) hours of occurrence. If your injury is work related contact University Human Resources, do not complete this form.
