Complete the OU SON Student COVID Exposure Form immediately: https://forms.gle/srUCz11PuLe96H1FA if you have not already done so.

Failure to complete the SON Student COVID Exposure Form may result in an unexcused absence and may also result in a delay for you to return to campus and/or clinical.

Notify your course FOR and your clinical faculty (if a clinical course) immediately in writing via email if you have not already done so.

If you are scheduled for simulation, notify the Simulation Coordinator ([email protected]) immediately in writing via email if you have not already done so.

Review the SON COVID-19 Information and Policies.

Wait for further instructions from SON COVID. Begin the isolation process immediately.

After receiving your email and the completed OU SON Student COVID Exposure Form, SONCOVID will follow up with you with instructions you need to follow regarding isolation, testing, and return to campus and will also provide you with COVID-related resources.
